Join us for an online workshop designed for Ontario residents on Advance Planning for your Future Health Care on Monday, June 10 from 10:00 a.m. – 12:30 p.m.

Are you prepared for a change in your health, such as an accident or a stroke, which leaves you unable to communicate? Do you have a Power of Attorney for Personal Care appointing someone to speak on your behalf? Does your Substitute Decision-Maker know your specific wishes in dealing with treatment decisions on your behalf?  

Register for this comprehensive Advance Health Care Planning workshop which will help you record your preferences for medical intervention in emergency or end-of-life care. We will review the responsibilities of the Substitute Decision-Maker and Ontario Power of Attorney legislation.

There will be an opportunity to discuss issues in small groups and to ask questions.

Presenters: Your workshop facilitators are Jon Snipper, JD, and Beverlee McIntosh, MSW, volunteers with DWDC’s Ottawa Chapter. Their professional backgrounds are in law and social work respectively.
