Join us for an online workshop designed for Ontario residents on Advance Planning for your Future Health Care on Thursday, October 12 from 1:00 – 3:30 p.m. 

Are you prepared for a change in your health, such as an accident or a stroke, which leaves you unable to communicate? Do you have a Power of Attorney for Personal Care appointing someone to speak on your behalf? Does your Substitute Decision-Maker know your specific wishes in dealing with treatment decisions on your behalf?  

Register for this comprehensive Advance Health Care Planning workshop which will help you record your preferences for medical intervention in emergency or end-of-life care. We will review the responsibilities of the Substitute Decision Maker and Ontario Power of Attorney legislation.

There will be an opportunity to discuss issues in small groups and to ask questions.

Registration is limited. A waitlist will open once the number of participants reaches capacity. Please email ottawa.events@dyingwithdignity.ca before Friday, October 6 to request to be added to the waitlist. We will not accept registration after Friday, October 6. 

Preparation: After your registration has been confirmed, you will receive an email with useful materials to prepare you to maximize the benefits of the workshop. We also urge you to download your Advance Care Planning Kit.

Speakers: Your workshop facilitators are Jon Snipper, JD and Beverlee McIntosh, MSW, volunteers with DWDC’s Ottawa Chapter. Their professional backgrounds are law and social work respectively. 

Cost: There is no cost to attend. However, any donation you choose to make to DWDC is much appreciated. Make a contribution

This workshop is designed to help you decide who you could ask to serve as your Substitute Decision-Maker (SDM). If you already have an SDM, we encourage you to invite the person who will serve as your SDM to register with you for this workshop.
