Join us for an online workshop designed for Ontario residents on Advance Health Care Planning on Wednesday, February 8 at 1:00 p.m. EST.

Are you prepared for an unexpected major catastrophe to your health, such as a bad accident or a stroke, which leaves you unable to communicate your health care treatment instructions, even temporarily? Do you have a Power of Attorney for Personal Care (POAPC) appointing someone to speak on your behalf when you can’t, called a Substitute Decision-Maker (SDM)? If so, do they know your specific wishes in dealing with various kinds of medical issues that might occur, or the kind of results from (non) treatment you are looking for?    

This workshop will focus on preparing for the possibility of losing the capacity to make or express decisions about your health care, before or near the end of your life. It is designed to help you select your Substitute Decision-Maker, and clarify what that person needs to know about your wishes when you can no longer express them.

ZOOM: You do not have to be a ZOOM subscriber to attend this workshop. Before the meeting, you will be sent an email with the link that you will tap to join the meeting. However, you may find it useful to join ZOOM for free as a member (zoom.us) and learn the basic usage.

Preparation: Soon after your registration has been confirmed, you will receive an email outlining the materials you need to review and what you should prepare to maximize the benefits of the workshop. Download your Advance Care Planning Kit.

Speakers: Your workshop facilitators are Jon Snipper, JD and Beverlee McIntosh, MSW, volunteers with DWDC’s Ottawa Chapter. Their professional backgrounds are law and social work respectively.

We encourage you to invite the person who will serve as your Substitute Decision-Maker (SDM) to attend the workshop as well, assuming they have agreed to act in that capacity. Of course, this workshop is designed to help you decide who you could ask to serve as your SDM, if you don’t know yet.
