Request an Advance Care Planning (ACP) presentation for your community

Everyone should have an Advance Care Plan, no matter their age or current health status.

Advance Care Planning is a process where an individual:

  • Thinks about their values, beliefs, and preferences for future health care.
  • Writes down (or “documents”) their directions in an Advance Directive.
  • Shares these directions with a Substitute Decision-Maker that they choose.

It can be difficult to know how to start the process of Advance Care Planning. That’s where Dying With Dignity Canada comes in.

Our staff and volunteers across Canada may be able to offer your community an in-person or virtual presentation, depending on capacity and availability.
