Making a plan for end of life and then sharing it — it’s maybe the best thing you can do to help ensure the treatment you want and the peace of mind you need.

Why is it, then, that only one in seven Canadian adults has written an Advance Care Plan, a document that communicates their wishes for care when they can no longer speak for themselves?

At Dying With Dignity Canada, we don’t have all the answers, but we do have a plan to turn the tide. With interest in end-of-life issues at an all-time high, we’re launching a national effort to engage Canadians to think more about — and ultimately, lay out — their choices.  And we’re calling the campaign What’s in Your Plan?

Will you take the Pledge to Plan?

Life can change at the blink of an eye. There’s simply no better time than the present to plan for peace of mind — for you and for the people in your life. By pledging to participate in What’s in Your Plan?, you will be saying YES to empowering yourself and others and NO to leaving your future up to chance.

Opportunities to act

Thinking about your wishes for end of life is one thing, but finally putting them into a plan is another. That’s why What’s in Your Plan? will feature tools to help you:

  • Make an Advance Care Plan that reflects your values and choices, if you haven’t already;
  • Share your wishes with the loved ones and healthcare professionals who may be responsible for making decisions about your care;
  • Defend your plan, by asking your provincial decision-makers what they’re doing to ensure your end-of-life rights are respected.

Once you’ve made the Pledge, please share it with friends and family to let them know about the campaign. Not only is sharing your plan essential to you achieving your goals, it’s a great way to encourage others to accomplish theirs, or at least start the conversation.
