The Ottawa Chapter will be holding an Advance Care Planning (ACP) workshop for DWDC members and supporters on May 14, 2016 from 1:00 to 4:30 PM. As these workshops are popular, we encourage you to register early. And consider coming with the family member or friend who may be your Substitute Decision Maker (as designated in your Power of Attorney document). The workshop is a great starting point for a discussion about your end-of-life wishes.
The session will take place at the Ottawa Archives (James Bartleman Centre) located in Centrepointe at 100 Tallwood Drive, around the corner from Woodroffe Avenue. Although there is parking at the back of the building, we encourage you to take public transportation, i.e. OC Transpo buses 94, 95, 156, 172, 174 and 178. PLEASE NOTE AND RESPECT THAT THIS FACILITY IS SCENT AND SMOKE FREE.
