How to access assisted dying in the Ottawa region

HOW TO REQUEST MEDICAL ASSISTANCE IN DYING (MAID) IN THE OTTAWA AREA

Your first step is to discuss your situation and the possibility of requesting MAID with your health care provider.  A health care provider who does not want to discuss this with you, is required to refer you to a physician or nurse practitioner who will do so. More information on that requirement is available here.

If you are not comfortable talking with your provider or do not have one, contact the Champlain Regional MAID Network maid@toh.ca

Your application form requires that your signature be witnessed. DWDC Ottawa Chapter can help you meet that requirement.  See the section on the Ottawa Chapter page about Independent Witnesses.
