Advance Care Planning is the process of thinking about, talking about, and documenting your wishes for health care in the event of serious injury or illness. The name of the document varies from province to province, but in Alberta it is called a Personal Directive. All Albertans over the age of 18 should have a Personal Directive.
On October 23 at 1:00 PM MST, Alexandra Kushliak from Alberta Health Services will introduce Albertans to the concept of Advance Care Planning, Personal Directives and Goals of Care during a one-hour webinar. Resources will be provided and Alexandra will answer questions during a Q&A period at the end of the session.
