You are invited to attend a free workshop to introduce you to the Advance Care Directive, presented by local members of the Dying With Dignity Canada Toronto Chapter.
The Advance Care Directive is the document in which you can make known your wishes for medical treatment or non- treatment. It is written while you are competent and have a vision of your own life. No one other than yourself can alter it. It will help those responsible for your care to decide on your treatment and help your loved ones make healthcare decisions in the event you cannot speak for yourself.
It covers such topics as “Considering your Personal Values” and “Considering your Medical Priorities”. These are discussed in thoughtful detail throughout the evening.
Knowing your rights as a patient will help you make crucial decisions about your future medical care. Learn how to develop an Advance Care Plan that is consistent with your end of life instructions.
