Making your Choices known through an Advance Care Directive
When you become a member of Dying with Dignity, one of the benefits is to receive an Advance Care Directive resource for your personal use.
An Advance Care Directive, sometimes referred to as a “Living Will”, is used to document your personal care wishes, if and when you can’t make those decisions for yourself.
Advance care planning is important because it gives you the opportunity to make choices about your future personal care, including:
- health care (treatment and services)
- food
- living arrangements and housing
- clothing
- hygiene
- safety
While advance care directives will vary from person to person, the process requires careful thought and communication. We encourage you to talk things over with people who are close to you.
As another member benefit, Dying With Dignity provides assistance in completing advance care directives and offers support on how to talk with family members, friends and health care providers about your wishes.
